Use this form to identify where you want your IRS Cash Refund to be deposited, and also your Consulting Success Fee to be deducted, once you've received your Cash Refund from the IRS.
Bank Information
AUTHORIZATION:
I authorize American Development Group, Inc DBA American Made Business Resources (“Consultant”) to process an ACH on bank account in the agreed amount listed above from my bank account provided herein and applied toward my outstanding balance owed to Consultant. I agree that I will pay in accordance with the issuing bank holder agreement. I acknowledge and agree to pay Consultant for any and all insufficient charges it may incur if my account is overdrawn. I understand and acknowledge that Consultant’s Success Fee may be increased due to interest charges or bank charges.
